After you place your order, please fill out the order form. I prefer a hard copy be mailed with your items, but if you don’t have a printer you can email a completed form to daisiesandmemorials@gmail.com or fill out the Google Doc version.

As long as it’s dry and fits, I can include it! Here are some examples to give you ideas. – Ashes

– Small dog harness or t-shirt (<15 lbs)

“I’m not comfortable sending in my items. Can I still have a pyramid made?”
YES! Personal items are not required to memorialize your pet! You can send me pictures digitally, and I can make you a pyramid with those.
